Psychiatry and Behavioral Sciences of the Johns Hopkins Medical Institutions
Program Objective
Targets public housing residents with a specific
objective of getting 75% of individuals who require ongoing care into the formal mental
health system within six months.
Outreach Model
The PATCH Model consists of the following steps:
- Identify public housing sites with high rates of
emergency care and hospitalization.
- Psychiatric nurses provide education and training for
public housing staff.
- Indigenous housing workers such as janitors and managers
make referrals to the nurse.
- The nurse offers and performs an assessment in the
resident's home.
- A psychiatrist makes a home visit with the nurse.
- As necessary, a treatment plan is developed
collaboratively with the resident.
- The nurse provides initial treatment and support with
backup from the psychiatrist.
- The nurse provides case management during a gradual
transition to mainstream care.
