Program Objective

To identify, assess, and engage elderly individuals who are in need of but reluctant to use services, and to educate, assist, and gradually link them to support and treatment.

Outreach Model

Outreach engagement techniques may include telephone contact or visitation to the older adult's home or other location. The engagement process is based on the following principles:

• Outreach workers carry out a program to build community awareness and referral sources.Eligibility is not dependent on payer, transportation, location, or acceptance of services.An individualized approach ensures that clinician visits are comfortable for the client.Outreach is consistently provided by the same clinician to establish client rapport.
• Clinicians provide information about mental health services and community resources.
