Program Objective
The objective is to fully integrate the outreach and delivery of primary health care and behavioral health care for older adults.
Outreach Model
The general principle of MAP is to coordinate and improve service delivery between and among the major programs that provide services to older adults. One of the outstanding benefits is that human service entities are able to strategize and combine outreach efforts and resources in an effective and coordinated manner. Resources are used in a comprehensive and coordinated way to address primary health care, mental health, and substance abuse community education needs.
