Salt Lake City, Utah

a community mental health center-based program

Program Objective

The objective is to reach out to older adults who have symptoms of depression and/or anxiety and help them learn to manage their illness through a partial hospitalization program.

Outreach Model

The Masters program works very closely with other agencies and programs serving older adults to assess individual mental health and other needs and coordinate services to meet those needs. Significant time and effort is directed towards reaching individuals who may not otherwise be connected to mental health services of any kind. Educational sessions are offered on a regular basis to home health agencies, hospitals, senior centers, day care, aging services, adult protective services, residential care and housing organizations, and others. Active interagency collaboration and education facilitates communication and expedites identification of those in need.
