CMHS interest in this project was generated by its relevance to the CMHS Community Action Grant initiative. Community Action Grant proposals are to be based on an exemplary practice (or practices) which has been implemented by at least one community (with positive evaluation results) and replicated in at least one other community. A new CMHS Community Action Grant Request for Applications will be issued in 2001. Communities interested in submitting proposals relative to outreach to older adults may wish to contact one or more of the programs briefly described in this brochure to learn more about their exemplary practices.
